Records of completed immunization must be filed with 黑料正能量鈥檚 Health Office in order to safeguard your health and the health of the entire community. As prevention is key to avoiding risk, you are expected to provide proof of the following immunizations as a student at 黑料正能量:

• Measles, Mumps and Rubella (MMR)
• Tetanus 鈥� Diphtheria or DPT
• Meningococcal Vaccine (A, C, Y, W-135):聽called聽Menactra or Menveo, should be done within the past 5 years

The penalty for failing to comply with our immunization policy is administrative suspension, which would prevent you from enrolling in the following semester.

Please refer to the聽Student Immunization Record聽[docx]聽form or the 黑料正能量 Health Office for further details. 黑料正能量鈥檚聽immunization compliance deadline is聽August 19 for the fall semester and December 31 for the spring semester.聽Students who wish to apply for an exemption, please fill out the聽Immunization Waiver Request [doc].听
